Step II: Submit the Required Documents

  • A copy of the completed VFS Global Indian Passport Renewal USA Order Form.
  • A completed document checklist with the applicant’s signature. 
  • In order to apply online, a user must fill out and sign the application form. 
  • Three copies of the Nationality Verification form with three pictures of the applicant. 
  • Three recently clicked pictures of the applicant. If the passport has been expired for more than 3 years, then the applicant must submit eight photographs. One photo must be glued to the provided space on the first page. The second photo should be glued to the Nationality Verification Form and the rest of the photos need to be stapled to the last page of the application form.

  • For female candidates, an affidavit of change of name from maiden to married must be notarized.
